There are many possibilities, some are just more probable than others. For example:Tom: where Jim had "had", "had had had had had had had" had had the teacher's approval.I'm not sure in what situation a teacher would approve "had had had had had had had," but if we ask Tom, maybe he'll tell us. :)
Tom, where Jim had had "had," had had "had had." "Had had" had had the teacher's approval.(Although I really believe one ought italicize where we have quoted.)
